New Patient Registration

How to register

We have an open list and welcome requests for registration from patients living in or moving to the N9 practice area.

You can register by completing a form available from reception or register by our online registration service tab.

 You will need to complete GMS1 and health questionnaire forms . You will be required to provide proof of address and identification to register.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

When you register, our Health Care Assistant will wish to see you to conduct a New Patient Medical.

We normally prefer you to see the same doctor as this ensures continuity of care. When your own doctor is not available, you may be asked to see one of the partners.

It would be helpful if you could bring any medical information with you e.g. list of medications/Red Book and family history of illnesses. Please arrive 15 minutes earlier than your appointment time.

Medical treatment is available from the date of registration. Please contact reception for further information.

Please could you bring in the RED BOOK for any children you are registering.

Changes to Personal Details

Patients are advised that it is their responsibility to inform the surgery of any changes in personal details. You are required to put this in writing and give to the receptionist. We will then update your medical information. Proof may be required for some changes that are made.
